OK, it was HQ on Bog, I was using my Barrett 50.cal, and a enemy sniper was near. I knew a enemy had a Acog M40, so I threw my smoke, and ran through. To my utter surprise, I got knifed in the middle of it, watching the killcam, turns out he didn't even see any smoke! Saw me right through!

Any explanation as to how this happened, please speak up.

It's just a glitch with the other persons 360. Sometimes their 360 will not render something like an explosion or a smoke grenade. At least that's what I think it is, I have had a few things happen like that where I saw smoke from an explosion and someone who shot me didn't see any.

I'm hoping they change how spawns work soon, I'm tired of enemies spawning right next to me

Good aggressive tactical move.... Sadly you where let down by a glitch. The same type of glitch that allows me to be killed by a skorpion halfway across the battle field yet the three .50cal rounds i pumped into him before hand barely made a scratch??? It's a crazy make believe world heheheh!

Did you wait til the smoke got nice and thick? Sometimes it takes a few seconds and people are clearly visible at that time.

I've actually had the opposite happen to me. I saw a person throw a smoke grenade (I knew it was a smoke because i heard the sound). I also knew it was next to me, but I didn't see any smoke, and I killed him when he ran in. He later accused me of hacking. But yes, there are a lot of glitches that need to be fixed. (damn grenades)

Thanks for the fast replies, I guess this game is pretty bugged after all. If anyone wants to maybe get a game of TDM, tactical style, throw a Request to SavageFlameFIST, I'm rank 51, in first prestige, not planning to do it again. Also, I was in cover when I threw the smoke, and I definitely saw it get pretty thick.
